The Future Health NOW Foundation (“FHN” or “we”) is an independent, non-profit private foundation that works with communities to create conditions for vulnerable children so they can realize their full potential in school, work and life.
To support and facilitate our work, FHN offers the Future Health NOW Foundation Portal, located at https://www.futurehealthnow.org/community (the “FHN Portal“), the Future Health NOW Foundation Expanding Equity Platform (the “FHN Platform”) and the websites operated by FHN that link to this policy, including those located at https://www.futurehealthnow.org, and other select microsites and landing pages, unless the website indicates that another privacy policy applies (collectively, the “FHN Websites”, and together with the FHN Portal and FHN Platform, the “FHN Web Applications”). This privacy policy (“Privacy Policy“) explains what Personal Information FHN collects from you via the FHN Web Applications and how we use and disclose that Personal Information. “Personal Information” is personally identifiable information or other like terms to mean any information that directly or indirectly identifies you or is reasonably capable of being associated with you or your household. Personal Information does not include: (1) information regarding your organization that is not specific to an individual, such as your organization’s annual revenue or budget; (2) publicly available information or lawfully obtained, truthful information that is a matter of public concern; and (3) de-identified and aggregated data that is not capable of identifying you. If we use de-identified or aggregated data, we commit to maintain and use the information in a non-identifiable form and not attempt to reidentify the information, unless required or permitted by law.
By visiting or using the FHN Web Applications, by pursuing or receiving a grant, program-related investment or contract from us, or by otherwise providing Personal Information to us, you acknowledge having read this Privacy Policy. In addition, this Privacy Policy is to be read together with, and forms a part of, any terms of use applicable to a FHN Web Application.
2. What Personal Information Do We Collect?
We may collect the following Personal Information when you visit or use the FHN Web Applications.
A. Personal Information that You Provide to Us
FHN may collect Personal Information directly from you. This may happen in several ways:

Register for an Account. You may provide us with Personal Information when you register for an account on the FHN Portal, the FHN Platform, or another FHN Web Application. Such information may include, but is not limited to, your name, organization affiliation, postal address, phone number, email address, username, password, photo, location, personal bio, and preferred language.

Pursue or Receive a Grant, Program-Related Investment or Contract. You may provide us with Personal Information when you explore, pursue or receive a grant, program-related investment or contract. Such information may include, but is not limited to, your name, title, organization, postal address, phone number, mobile number, email address, and tax ID number.

Sign Up for Communications. You may provide us with Personal Information when you sign up for communications from FHN. Such information may include, but is not limited to, your name, company or organization affiliation, postal address, email address, and phone number.

Download Resources. You may provide us with Personal Information when you request to receive or download publications or other resources through the FHN Web Applications. Such information may include, but is not limited to, your name, company or organization affiliation, postal address, email address, and phone number.

Sign Up for Events. You may provide us with Personal Information when you sign up for our events, such as workshops, conferences, and webinars, either through the FHN Web Applications, through our service providers, or offline. Such information may include your name, company or organization affiliation, postal address, email address, and phone number.

Contact Us. You may provide us with Personal Information when you contact us directly with your inquiries or comments.

Apply for a Job. You may provide us with Personal Information when you apply for employment with us through our FHN Websites.

Participate in Online Courses. You may provide us with Personal Information when you participate in online courses through the FHN Platform or other FHN Web Applications, such as by participating in discussion boards, completing polls, and submitting assignments.
B. Information Collected from Third Parties
FHN may also collect information from certain third parties, such as Guidestar. Such information may contain information regarding your organization (e.g., its mission) and limited Personal Information (e.g., name of your organization’s executive director).
C. Information Automatically Collected by FHN
If you visit the FHN Web Applications, FHN may automatically collect certain information from your device, such as the following:

Your usage and activity on the FHN Web Applications and other third party websites (such as the pages you visit);

Geographic and demographic data; and

The amount of time you spend on the FHN Portal.
This information may be collected automatically using log files, cookies, pixels, or other tracking technology. This automatic data is used for the administration of the FHN Web Applications, for statistical analysis, to monitor the use or performance of the FHN Web Applications, to ensure that the content on the FHN Web Applications is presented in the most effective manner for our visitors and users, and to track users across the websites and services of other businesses for targeted advertising.
3. How Do We Use Personal Information?
We may use your Personal Information for the following purposes:

4. How Do We Disclose Your Personal Information?
We may disclose your Personal Information with the following third parties:

with our service providers that host the FHN Web Applications, such as Fluxx Labs Inc.;

with our other vendors, consultants and service providers that provide services on our behalf, such as translators, evaluators, technology platform providers and data hosting providers; and

with third parties in connection with the uses described in Section 3 above, such as with government or regulatory authorities in order to comply with laws
In addition, in connection with grants and program-related investments that we award, we may disclose your organization’s information generally on our FHN Websites, such as in a profile regarding your organization and its mission, in our annual report, and in other publications.
5. Email Communications
If you have signed up to receive newsletters or other communications from FHN, you may, at any time, inform us that you do not want FHN to send you such communications by opting out using the link at the bottom of each email. After opting out, you will no longer receive communications from the mailing list from which you opted out; however, you may still receive transactional or other communications from us (e.g., communications regarding a grant, program-related investment or contract).

7. Safeguarding Your Information
FHN cares about the security of your Personal Information. However, the Internet is a global communications vehicle open to threats, viruses, and intrusions from others. For this reason, FHN cannot promise, and you should not expect, that we will be able to protect your Personal Information at all times and in all circumstances. FHN cannot guarantee the security and privacy of transmissions via the Internet, and we will not be liable for any lack of security relating to the FHN Web Applications or any other communication over the Internet. To the maximum extent permitted by applicable law, FHN shall not be liable for any damages resulting from any loss of privacy or security occurring in connection with any such communications.
8. Third Party Analytics and Advertising Providers
A. Do Not Track Disclosure
Certain web browsers may provide an option by which you may ask your browser to inform websites you visit that you do not wish your activities to be tracked by cookies or other persistent identifiers, commonly called “Do Not Track Signals.” The FHN Web Applications currently do not respond to Do Not Track Signals.
B. Analytics and Advertising Providers
We allow others to provide analytics services and serve advertisements on our behalf across the web and in mobile apps. These entities may use cookies, pixels, device identifiers, and other technologies to collect information about your use of the FHN Web Applications and other websites, applications and services, including your IP address, device, web browser, mobile network information, pages viewed, time spent on pages or in mobile apps, links clicked, conversion information and how you got to the FHN Web Applications. This information may be used by us and others to, among other things, analyze and track data, determine the popularity of certain content, deliver advertising and content targeted to your interests on the FHN Web Applications and other websites, and better understand your online activity.
For example, we work with analytics service providers, including but not limited to Google Analytics, to help us better understand how our users use the FHN Web Applications. As a result, these parties may collect certain information from you about your online activities over time and across different websites when you use the FHN Web Applications. We also work with third parties to serve ads to you as part of customized campaigns on third-party platforms (such as social media platforms and search engine providers). As part of these ad campaigns, we or the third-party platforms may convert information about you into a unique value that can be matched with a user account on these platforms to allow us to learn about your interests and serve you advertising that is customized to your interests. Note that the third-party platforms may offer you choices about whether you see these types of customized ads.

9. Third Party Websites
The FHN Web Applications may contain links to third party websites. By accessing and/or using a linked third party website, you may be disclosing your Personal Information to such linked website. Therefore, you should carefully review the privacy policies of these other websites. If you are not comfortable with the policies of a linked website, you should not access and/or use that website. FHN is not responsible for the policies (privacy or otherwise) or the content, services or products offered by or through any third party website. It is solely your responsibility to review the privacy policy and other policies of third party websites to confirm that you understand and agree with their policies.
10. Exclusions
If you do not reside in the U.S. and are using the FHN Web Applications, or if you are pursuing a grant, program-related investment or contract with us, or if you provide Personal Information to us, it may be transferred to and/or processed on computers in the U.S. and other countries. The data protection and privacy laws of such other countries, including the U.S., may not afford you the same level of protection as those in your own country. Do not provide your Personal Information to us if you do not want this information to be transferred and/or processed outside of your country, or if the laws in your country restrict these types of transfers. Your provision of Personal Information to us will be handled in accordance with this Privacy Policy. By interacting with FHN, visiting the FHN Web Applications, pursuing or receiving a grant, program-related investment or contract with us, or by otherwise providing Personal Information to us, you agree to the terms of this Privacy Policy and are consenting to the transfer, storage, and processing of your personal information to and within facilities located in the U.S. and other facility locations selected by FHN.
11. Children Under 16
We do not seek to collect, nor do we knowingly collect, information from children under the age of 16. If a child has provided us with Personal Information, a parent or guardian of that child may contact us to have the information deleted from our records. From time to time, we may update this Privacy Policy. If we do, we will note near the top of this page the date that any changes are made and/or when they become effective. Your continued use of the FHN Web Applications following the effective date of a change to this Privacy Policy signifies your acceptance of and agreement to the terms of the revised Privacy Policy. FHN encourages you to regularly review this Privacy Policy so that you remain updated as to any changes.
INTERNATIONAL DATA PROTECTION RIGHTS
The laws of several countries, including and those in the United Kingdom and the European Union, grant their residents additional rights regarding use of their personal data as defined under those laws. The FHN Platforms are not intended for use by individuals located in the United Kingdom or European Union.
